ALL IN SPORTS AND EDUCATION
WAIVER AND RELEASE OF LIABILITY

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.

In consideration for the minor child identified below (“Participant”) being permitted to participate in any class or activity offered by All in Sports and Education, I, the undersigned parent or legal guardian (“Guardian”), hereby acknowledge, understand, and agree to the following:

1. Assumption of Risk

I acknowledge that participation in sporting activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, concussions, and other potential hazards that may result in serious personal injury, permanent disability, property damage, or even death. I voluntarily assume all such risks on behalf of the Participant.

2. Waiver and Release

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless All in Sports and Education, its officers, directors, employees, coaches, volunteers, contractors, agents, landlords, lessors, vendors, sponsors, and affiliated organizations (“Released Parties”) from any and all liability, claims, demands, losses, or damages arising from or related to participation in the Activities, whether caused by negligence of the Released Parties or otherwise.

3. Medical Authorization

I authorize All in Sports and Education and its representatives to obtain medical treatment for the Participant in the event of an emergency. I understand that I am responsible for any and all medical expenses incurred.

4. Concussion Policy

I acknowledge that I have been informed of the risks associated with concussions and other head injuries in youth sports. I agree to comply with all concussion-related safety protocols and understand the importance of timely reporting of symptoms.

5. Photo/Video Release

I grant All in Sports and Education the irrevocable right and permission to use photographs and/or video recordings of the Participant taken during the Activities for promotional, educational, and commercial purposes in print, online, and in other media. I waive any right to royalties or compensation.

6. Governing Law

This Waiver and Release shall be governed by and construed in accordance with the laws of the State of California. Any legal action or proceeding shall be brought exclusively in a court of competent jurisdiction located in California.

7. Severability

If any provision of this Waiver and Release is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I have read the above waiver, indemnification, and release of liability agreement, and understand that I give up substantial rights by signing it and knowing this, sign it voluntarily. I further agree that no oral representations, statements, or inducements, apart from the above agreement have been made. I agree that this agreement is intended to be as broad and inclusive as is permitted by law and if any portion is held invalid, it is agreed that the balance shall, notwithstanding, continue to have full force and effect. I agree to participate knowing the risks and conditions involved and do so entirely upon my own free will. I also acknowledge that if English is not my first language, that I have sought out the assistance of someone competent to translate this form to me. My signature below shall be a declaration that I have so read and understand this Waiver, Indemnification, and Release of Liability Agreement.
